Having done it faithfully for quite some time, I'd say the best results (thus far) I've gotten are:

-+3 Hat.
-Mouth Candy lower headgear.
-Those rare "good" drops from OBB/OPB.
-Bloody Branch.

If you're asking if there's a +4 hat, if there is, it must be very rare.

Koma said there were, but yeah, the chance must be really low. =/ Don't think anyone has gotten anything else so far.

If so, I'll be curious to see who first gets the SUPER PUMPKIN HAT. Because +4 all stats would be pretty killer.

Then again, so much so, it's understandable why it's so rare. You don't just want to start popping out +4 to all hats.

Nah, I doubt they'd make a new hat for that, especially since they use officials anyway.
If anything, the hats are other Halloween related Items. I have my guesses, but as I haven't seen anyone wear anything yet.. =/
